The patient
A male patient in his 40s
Patient’s medical evolution up to now
He first had a hernia in his early 20s.
At one point, he became almost bedridden due to pain in his lower back and both lower limbs.
After that, he underwent laser treatment.
In his 30s, he developed back pain and numbness.
He came to the clinic to improve his symptoms.
Symptoms before treatment
It feels painful no matter what position he is in.
He feels pain when sitting for a long period of time.

Main pain areas: lower back, legs
Forward bending: Discomfort in both thighs all the way to the back of the lower legs
Backward bending: Possible
Coughing/sneezing: painful during excitability phase
Lasegue test: positive at 30 degrees
Standing on toes and heels: Possible

・Daily activities
Turning over in bed and standing up are no problem.
Severe pain when getting up and washing face.
Imaging and findings
- Presence of a transitional vertebra in (L6)
- L4/5 – herniated disc
- L3/4, 5/6, 6/s – disc degeneration and protrusion
The above are the likely cause of symptoms.
The above was confirmed by the imaging.
Treatment
After consultation with the patient, the Cellgel procedure was performed on L3/4, 4/5, 5/6, and 6/s
The treatment was performed by Dr. Maruyama.
The treatment took about 60 minutes.
After resting in the recovery room, the patient was able to walk home by himself.

Risks and side effects of the Cellgel method: Transient pain may occur after treatment. Nerve damage is not a zero possibility due to the very nature of the treatment, but there have been no reports of damage so far in either report cases or publications. There is a very small possibility of allergic reactions to local anesthetics. Symptoms may temporarily worsen during the first week or two after treatment. This is believed to be due to the decompression effect of the implant, which retracts the surrounding tissues. In addition, if the disc is almost completely collapsed, treatment may be difficult. The doctor hold a consultation with you during your visit to the clinic to determine the best treatment option for your condition.
