Column Treatment Case Report No. 160| ILC International Lumbago Clinic (Osaka Clinic)

The patient

A female patient in her 70s.

The patient’s medical history to date

The patient was diagnosed with spinal canal stenosis at another hospital 3 to 4 years ago.

She was told that she would have to undergo surgery once she became unable to walk, and she is currently receiving oral medication for treatment.

Recently, she feels that her numbness has intensified.

The patient visited our clinic seeking to improve his symptoms.

Symptoms before treatment

Numbness is constantly felt from the lower back on the left side to the left toes.

Pain in the left buttock is aggravated in the supine position and by prolonged sitting.

Turning over in bed: no problem

Standing up: somewhat difficult

Washing one’s face: no problem

Prolonged standing and semi-crouched posture: somewhat difficult

Prolonged sitting: somewhat difficult

Lifting and carrying heavy objects: somewhat difficult

No pain at all is rated as 0, and the maximum possible pain is rated as 10.

Main areas of pain and numbness: lower back, lower limbs.

Bending forward: Sticking feeling in the back of the thigh

Bending backward: low back pain from the midline to the left side

Lassegue test: stiffness appear laterally on the thigh at a 70 degree angle on the left side

MMT: full

Lumbar point of tenderness: L3/4 and 4/5 on both sides

Coughing and sneezing: no aggravation

Imaging and findings

  • L1/2 – disc herniation
  • L4/5 – disc degeneration, spinal canal stenosis
  • L5/s – disc degeneration, disc herniation

The above findings were also observed on the imaging.

Findings show that the compression of the spinal canal on discs L1/2, 4/5, and 5/s is the probable cause of the symptoms.

Treatment

After consulting with the patient, the Cellgel Method was performed on L1/2, 4/5 and 5/s.

The treatment was performed by Dr. Maruyama.

The treatment took about 35 minutes.

After resting in the recovery room, the patient was able to walk home by herself.

Risks and side effects of the Cellgel method: Transient pain may occur after treatment. Nerve damage is not a zero possibility due to the very nature of the treatment, but there have been no reports of damage so far in either report cases or publications. There is a very small possibility of allergic reactions to local anesthetics. Symptoms may temporarily worsen during the first week or two after treatment. This is believed to be due to the decompression effect of the implant, which retracts the surrounding tissues. In addition, if the disc is almost completely collapsed, treatment may be difficult. The doctor hold a consultation with you during your visit to the clinic to determine the best treatment option for your condition.
