Column Treatment Progress Report No. 212: A Patient Who Had Undergone Multiple Surgical Procedures

The patient
A male patient in his 80s
Treated in September 2025. Approximately 9 months have passed since treatment.
Symptoms and treatments to date
The patient was diagnosed with spinal canal stenosis at L4/5, as indicated by the red box in the image.
Pre-treatment medical history and treatments to date
Twenty years ago, the patient underwent surgery for spinal canal stenosis, after which his symptoms improved.
His symptoms worsened again approximately 10 years ago, and he underwent decompression surgery 7 years ago, which again resulted in improvement.
Approximately 5 years ago, he began experiencing lower back pain again. He was advised that further surgery would be difficult due to his age.
He tried massage, acupuncture, and moxibustion, but his symptoms did not improve significantly.
After learning about our clinic, he visited us and underwent treatment.
Post-treatment progress
The table below shows the results of the patient’s own evaluation of the pain in each body site on a scale of 0 to 10.
No pain at all is rated as 0 and 10 as the maximum possible pain.

The following is feedback from the patient when contacted for follow-up.
1 week after treatment:
Overall, his symptoms had decreased.
1 month after treatment:
He had been hospitalized for another illness and as a result was not very active, so he was not experiencing symptoms.
3 months after treatment:
Washing his face in the morning after getting up was difficult, but walking had become easier.
6 months after treatment:
He was able to live his daily life without symptoms.
With this patient, the symptoms were considered to be primarily caused by spinal canal stenosis.
The patient’s symptoms worsened three months after treatment because he had been hospitalized for another illness.
After discharge, he also underwent rehabilitation at our clinic with the goal of improving his remaining symptoms and restoring his physical strength. As a result, his lower back condition improved and was better than it had been before his hospitalization.

Risks and Side Effects:
Temporary pain may occur after treatment.
Nerve injury is theoretically possible due to the nature of the procedure, but no such cases have been reported in our own experience or in published studies.
Extremely rare risk of allergic reaction to local anesthetic.
Symptoms may temporarily worsen for 1–2 weeks after treatment, believed to be due to decompression effects of the implant pulling on surrounding tissue.
If the disc is severely collapsed or nearly gone, treatment may not be possible.
The physician will discuss suitable treatment options with each patient depending on their condition.
