The patient
A male patient in his 70s.
The patient’s medical history to date
About a year ago, his iliopsoas muscle became stiff and stopped functioning properly, and he lost the ability to move his leg as a result.
After that, his leg began to hurt, and he experienced numbness in his buttocks.
He visited a different clinic where he was diagnosed with spinal stenosis.
He suddenly developed pain in his groin, which gradually evolved to lower back pain.
He is currently taking medication and monitoring his condition.
The patient visited our clinic seeking to improve symptoms.
Pre-treatment symptoms
Turning over in bed: Easy
Standing up: Slightly difficult
Washing his face: Easy
Maintaining a bent-forward or standing position: Slightly difficult
Sitting for long periods: Slightly difficult
Lifting and carrying heavy objects: Easy

No pain at all is rated as 0, and the maximum possible pain is rated as 10.
Main areas of pain and numbness: lower back to left lower limb.

Bending forward: Possible, although slight numbness occurs in the left lower extremity
Bending backward: No problem
Lasègue sign: Negative
Lumbar tenderness: Negative
Coughing/sneezing: Pain radiates when coughing strongly

Imaging and findings

- L2/3, L3/4: Disc degeneration, disc bulging, and foraminal stenosis
- L4/5: Disc degeneration, disc bulging, foraminal stenosis, and spinal canal stenosis
The above findings were also observed on the imaging.
Compression of the spinal canal associated with the disc abnormalities at L2/3, L3/4, and L4/5 was considered a likely cause of the patient’s symptoms.
Treatment
After consulting with the patient, the Cellgel Method was performed on L2/3, 3/4, 4/5 and 5/s.
The treatment was performed by Dr. Ohara.

The treatment took about 30 minutes.
After resting in the recovery room, the patient was able to walk home by his own means.

Risks and side effects of the Cellgel method: Transient pain may occur after treatment. Nerve damage is not a zero possibility due to the very nature of the treatment, but there have been no reports of damage so far in either report cases or publications. There is a very small possibility of allergic reactions to local anesthetics. Symptoms may temporarily worsen during the first week or two after treatment. This is believed to be due to the decompression effect of the implant, which retracts the surrounding tissues. In addition, if the disc is almost completely collapsed, treatment may be difficult. The doctor hold a consultation with you during your visit to the clinic to determine the best treatment option for your condition.
