The patient
A male patient in his 60s.
The patient’s medical history to date
He developed sciatica about 8 years ago, but so far he had been able to manage his condition through muscle training and stretching.
After playing golf last year, he started to experience pain and discomfort in his hip joints.
After that, since the pain intensified, he continued to do muscle training and play golf, but his condition did not improve as he had hoped.
He came to our clinic seeking treatment.
Symptoms before treatment
He had pain and discomfort in the entire hip joint area.
He had some dizziness when getting up after lying down.

Forward bending: easy
Backward bending: difficult
Pressure points: L3/4, L4/5, L5/S
Lasegue test: negative
Coughing and sneezing: no aggravation

・Daily activities
Turning over in bed and sitting for long periods of time are easy.
Holding a heavy object or bending over is somewhat difficult.
Imaging and findings
- L2/3, 4/5 – disc degeneration, protrusion
- L3/4, 5/s – disc herniation, spinal canal stenosis
The above findings were also observed on the imaging.
・The is a high possibility that the main symptoms are caused by the spinal canal being compressed due to the herniated disc at L3/4, 5/s and the narrowing of the intervertebral foramen, which is compressing the nerves.
There is also a possibility that the main symptoms are caused by the disc degeneration and protrusion in L2/3, 4/5.
Treatment
After consulting with the patient, the Cellgel method was performed on L2/3, 3/4, 4/5, and 5/s
The treatment was performed by Dr. Kodera.
The treatment took about 45 minutes to complete
After resting in the recovery room, the patient was able to walk home by himself.

Risks and side effects of the Cellgel method: Transient pain may occur after treatment. Nerve damage is not a zero possibility due to the very nature of the treatment, but there have been no reports of damage so far in either report cases or publications. There is a very small possibility of allergic reactions to local anesthetics. Symptoms may temporarily worsen during the first week or two after treatment. This is believed to be due to the decompression effect of the implant, which retracts the surrounding tissues. In addition, if the disc is almost completely collapsed, treatment may be difficult.
