The patient
A male patient in his 20s.
The patient’s medical history to date
The patient has had chronic low back pain since 2017. in 2021, while performing a deadlift, he developed a strained back, and numbness in the right leg appeared.
After a period of improvement, he consulted a local doctor based on the recommendation of people around him, where he was diagnosed with disc herniation.
He came to our clinic to improve his symptoms.
Symptoms before treatment
Difficulty in bending forward and backward as well as twisting around one’s body.

No pain at all is rated as 0, and the maximum possible pain is rated as 10.
Main area of pain and numbness: lower back, lower limbs
Forward bending: somewhat difficult
Bending forward: possible
Point of tenderness: L4/5 on both sides
Lasegue test: negative
Coughing & sneezing: Sometimes painful
Intermittent claudication: none
Additional explanations on our clinic’s medical examination
・Daily activities
The patient works as a farmer. Sustained standing or squatting posture is very difficult.
Sitting for long periods of time is no problem.
He used to play baseball and deadlift in the past.
Imaging and findings
- L4/5- Disc herniation
The above findings were also observed on the imaging.
・The disc herniation at L4/5 is likely to have compressed the spinal canal and caused the symptoms of pain and numbness.
Treatment
After consulting with the patient, the Cellgel Method was performed on L4/5.
The treatment was performed by Dr. Kotera.
The treatment took about 20 minutes.
After taking a rest in the recovery room, since the patient was able to sit, stand, and walk without any problems, he walked home on his own.

Risks and side effects of the Q-Florence method: Although there are no reports of such cases in our own experience or in the literature, there is a possibility of damage to nerves, dura mater, and other surrounding tissues due to insertion of the spacer. There is a very small possibility of allergic reaction to the local anesthetic. There is a possibility of dislocation or fracture of the spinous process. Temporary urinary problems and allergic reactions may also occur.
