The patient
A male patient in his 30s
Patient’s medical evolution up to now
13 years ago, he underwent surgery for a herniated disc. At that time, he had symptoms on the right side.
After that, he had no more symptoms, only occasional pain.
Since March of this year, his symptoms have not eased at all, and have been worsening instead.
He came to the clinic to improve his symptoms.
Symptoms before treatment
Pain when walking and standing.
He could hold a standing position for more than 30 seconds.

No pain at all is rated as 0, and the greatest possible pain is rated as 10.
Main pain area: legs
Forward bending: possible for now
Backward bending: impossible due to apprehension
Coughing/sneezing: pain-inducing
Lasegue test: left hip is painful at 30 degrees on the left and right side
Tenderness point: right L4/5,5/s
Heel stand: reduced on left leg
Standing on tiptoe: Possible

・Daily activities
Washing one’s face is Difficult.
Sitting is possible, but symptoms worsen when standing.
Imaging and findings
- L4/5- Disc herniation
The above is confirmed by the imaging.
Spinal canal compression due to herniated L4/5 disc is very likely the cause of the left-sided predominance of symptoms.
Treatment
After consultation with the patient, the Cellgell procedure was performed on L4/5.
The treatment was performed by Dr. Maruyama.
After the DiscoGel injection
The treatment took about 25 minutes
After resting in the recovery room, the patient was able to walk home by himself.

Risks and side effects of the Cellgel method: Transient pain may occur after treatment. Nerve damage is not a zero possibility due to the very nature of the treatment, but there have been no reports of damage so far in either report cases or publications. There is a very small possibility of allergic reactions to local anesthetics. Symptoms may temporarily worsen during the first week or two after treatment. This is believed to be due to the decompression effect of the implant, which retracts the surrounding tissues. In addition, if the disc is almost completely collapsed, treatment may be difficult. The doctor hold a consultation with you during your visit to the clinic to determine the best treatment option for your condition.
