Column Case Report No. 63| ILC International Lumbago Pain Clinic (Osaka) Column

The patient:

A female patient in her 50s

The patient’s medical history to date

She was once unable to move when she was 20 years old, but the pain eased after a block injection.

After that, she began feeling pain in her lower back, but the symptoms really began to worsen from July this year.

She took medication and had block injections, but they had no effect. She was also advised to have surgery.

She came to our clinic seeking to improve her symptoms.

Symptoms before treatment

 

Dull pain in the lower back. The pain worsened when she bent over, and she felt coldness and heaviness in both lower legs.

Pain in both sides of the groin.

Forward bending: impossible

Backward bending: a little painful

Pressure points: predominant on the right side in L3/4, 4/5, 5/s

・Daily activities

 

Turning over in bed and lifting or holding heavy objects are somewhat difficult.

Walking is possible, but the patient feels fatigue.

 

Images and findings

 

  • L5/s – Disc degeneration, protrusion, slight narrowing of the left intervertebral foramen.

The above findings were also observed on the imaging.

・There is a high possibility that the protrusion of the L5/S disc is causing nerve compression and the symptoms in the lower back.

 

Treatment

After consulting with the patient, the Cellgel method was performed on L5/s.

The treatment was performed by Dr. Maruyama.

The treatment took about 55 minutes.

After resting in the recovery room, the patient was able to walk home by herself.

Risks and side effects of the Cellgel method: Transient pain may occur after treatment. Nerve damage is not a zero possibility due to the very nature of the treatment, but there have been no reports of damage so far in either report cases or publications. There is a very small possibility of allergic reactions to local anesthetics. Symptoms may temporarily worsen during the first week or two after treatment. This is believed to be due to the decompression effect of the implant, which retracts the surrounding tissues. In addition, if the disc is almost completely collapsed, treatment may be difficult. The doctor hold a consultation with you during your visit to the clinic to determine the best treatment option for your condition.
